Primary healthcare systems in various parts of Africa are facing increasing pressure due to a combination of factors such as rising demand, chronic staff shortages, and shrinking international aid budgets. In response to these challenges, artificial intelligence (AI) is being explored not as a revolutionary technology but as a means to sustain essential services.

A new initiative called Horizon1000, supported by the Gates Foundation and OpenAI, is set to introduce AI tools into primary healthcare clinics in multiple African countries. The project will kick off in Rwanda with the goal of reaching 1,000 clinics and their surrounding communities by 2028, backed by a $50 million investment.

This initiative comes at a critical time as global development assistance for health has decreased by nearly 27% compared to the previous year, leading to setbacks in progress and an increase in preventable child deaths. Horizon1000 focuses on leveraging AI for everyday tasks within under-resourced clinics, such as patient intake, triage, record-keeping, appointment scheduling, and access to medical guidance.

Bill Gates emphasized the potential of AI in expanding access to quality care, particularly in countries with severe health worker shortages and limited infrastructure. The collaboration between the Gates Foundation and OpenAI aims to support healthcare workers rather than replace them, with OpenAI providing technical expertise and AI systems, and the Gates Foundation overseeing deployment in alignment with national guidelines.

Rwanda was chosen as the pilot country due to its existing digital health initiatives and efforts to integrate AI into healthcare. The initiative aims to reduce administrative burdens, enhance care quality, and increase patient access through responsible AI implementation.
